BEFORE you utilize Powers with Animals services, please read our Policies and Procedures. PRINT out the appropriate forms and the applicable waiver(s) from the PDF documents located below. Read them carefully and please fill them out completely. THEN contact us to set up a time for the New Client Consultation.
Please have the forms filled out and ready by our meeting, so we have the information we need to take care of your friends. Feel free to add as much information as you would like: the more the better. Any questions or concerns, can be addressed when we come over.

Obedience Training Classes & Private Sessions-Please be sure to include what is going on, and what it is you would like to accomplish.
Pet Sitting-  Our "Meet & Greet" takes 30-60 minutes. (Unless we get chatty)
POWWA Dog Hiking Adventure Club-The "Meet & Greet" takes about 60 minutes including a one-on-one hike. In this way we can get acquainted with your animal(s).

Private Sessions
PRIVATE TRAINING SESSIONS- Client & Dog Information and Waivers.Besides basic obedience. this training can cover Behavior Modification for cats, dogs and other companion animals. Judy Powers' training is in the Ttouch Method.  She is a Practitioner 1. Ttouch can help with shyness, nervousness & anxiety as well as hyper-activity, reactivity & "puppy biting". Ttouch is a non-invasive method for helping with pain management, and old age issues.
Please spend some time thinking about what it is you want to accomplish with your animal. And remember, you are going to be learning how to teach your dog. The waivers are for Powers with Animals, and a private space that we may end up utilizing.
Obedience and Behavior Modification Classes
OBEDIENCE CLASS- Client & Dog Information and Waivers. The class covers basic obedience training, methods to teach your dog how to walk properly on a leash, and behavior modification skills. All of this in a controlled situation with other people and dogs.
Please spend some time thinking about the questions on the form. You will get more out of the class if you have a clearer idea of what your goals and expectations are. Also there is a waiver for the class AND a waiver for the woman who is graciously allowing us to use her property. Plus proof of current vaccinations required.
Dog Sitting During the Day- shorter version
PET SITTING DURING THE DAY- Client Information & Waiver- PLEASE READ THOROUGHLY AND FILL OUT COMPLETELY
This is for people who are visiting Moab or live here and would like Powers with Animals to watch their animals while they play/work during the day. Must have a place for us to come visit or stay with your animal.
Cat Sitting
 CAT SITTING- Client, Cat and House Information. Waiver- PLEASE READ THOROUGHLY AND FILL OUT COMPLETELY
This form is for those who are interested in having Powers with Animals take care of their cat(s) while they are out of town. The more complete the information, the better we will be able to care for your babies.
DOG SITTING- Client, Dog and House Information. Waiver- PLEASE READ THOROUGHLY AND FILL OUT COMPLETELY
Dog care- multi-day and overnights. This form is for those who are interested in having Powers with Animals take care of their dog(s) while they are out of town. The more complete the information, the better we will be able to care for your babies.
Dog AND Cat Sitting-extended stays/overnights
DOG & CAT SITTING- Client, Animals and House Information & Waiver- Please Read Through and Fill Out the Applicable Information. This form is for those who are interested in having Powers with Animals take care of their animals while they are out of town. The more complete the information, the better we will be able to care for your babies.

POWWA DOG HIKING ADVENTURE CLUB- Client and Dog Information. Waiver
If your dog would like to join the POWWA DOG HIKING ADVENTURE CLUB, Powers with Animals needs as much information as possible to ensure a good fit and that everyone has an enjoyable time hiking, smelling, etc.
The Waiver is included and a signature is required on both forms.
